Our practice collects some basic information in a registration form about the child or adolescent to create an electronic patient record and to have the correct contact details, as well as to match them with the appropriate coach or psychologist.
Specifically for minors, there are several conditions that our practice must adhere to. The consent form for the guidance of minors is a necessary document in this regard.
